Travel Vaccinations

If you require vaccinations relating to foreign travel you need to fill out a Travel Health Questionnaire. You can complete an online form below or request a copy from the Reception desk at the Surgery.  Once completed you can email it to us ([email protected]), via post or return it to the surgery (If it is more convenient to drop it off outside of our working hours you can put it in our secure postbox located outside of the surgery on the wall).

The Practice Nurse will check which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to complete and return the form as early as possible – at least 8 weeks before you travel – as any vaccines required will have to be ordered as they are not a kept in stock.

The vaccinations should be given at ideally 4 weeks before you travel to allow time for them to work. Please note that we are only able to give certain holiday vaccinations at the surgery, and you may be asked to attend a private/commercial travel clinic for the other vaccinations. The nurse will inform you of this as soon as possible. If a private prescription for malaria tablets is required, there will be a charge for this.
