If you require vaccinations relating to foreign travel you need to fill out a Travel Health Questionnaire. You can obtain this from the link below or request a copy from the Reception desk at the Surgery. Once completed you can email it to us (Somicb.millbrook.surgery1@nhs.net), via post or return it to the surgery (If it is more convenient to drop it off outside of our working hours you can put it in our secure postbox located outside of the surgery on the wall).
The Practice Nurse will check which countries and areas within countries that you are visiting to determine what vaccinations are required.
It is important to complete and return the form as early as possible – at least 6 weeks before you travel – as any vaccines required will have to be ordered as they are not a kept in stock. The vaccinations should be given at least 2 weeks before you travel to allow time for them to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
